Pressure Injury Foundations: Support Surfaces for Prevention  and Treatment

This application form is for external learners (non-current Culver-Stockton students)pursuing the digital badge in support surfaces for prevention and treatment of pressure injuries. If you are a current Culver-Stockton College student please register through Online Registration on the Student Tab of your MyCulver Account-do not use this form.

Complete your application for your digital badge by clicking “Next Page” (below). Once the application is processed you will receive your MyCulver username and temporary password via a text message. Once logged into your MyCulver account, you can change your password from the Home Tab – Change Your Password. 

Culver-Stockton College utilizes Canvas LMS and this badge or course can be accessed from a link on the main page of MyCulver (under Quick Links – you do not have to login to MyCulver to access Canvas). Your Canvas login will be the same username and updated password as you MyCulver.

Your balance will be due upon completion of this registration. The total coast will be $400. An additional $100 transcript fee will be applied if you elect to take this course for academic credit. You are not required to take this for academic credit, a digital badge will be awarded regardless upon satisfactory completion of the course materials.

• I must complete all materials at a satisfactory level within the course window. This includes module quizzes and final exam at 80% or higher. The final score/grade for completion of the course will be “Pass-P” or “Not Pass- NP”. P results in the awarding of the digital badge. No badge will be awarded for an NP.
• Any NP will require retaking the course, at the advertised cost, for the opportunity to earn a P or an awarded badge.
• Once registering the course cost and fee is non-refundable.
• This digital badge is not eligible for federal financial aid (external learners only).
